You have tried various sites thousands of times? Maybe then you should contact your bank... you're sure the expires-date and 'the secret number' are typed correctly?
One thing I have on my mind... just noticed that one site didn't approve my visa (perhaps because of my country). So, I joined
http://www.epassporte.com and got a virtual visa that works like a charm! Maybe worth a try... but first you need to register with to ePassporte with your old visa (which might become a problem).
